COMMUNITY CARE


Who is ERA's service for?

For any person in Victoria who:

  • has applied for or receives:
    • a Community Aged Care Package;
    • an Extended Aged Care In The Home Package;
  • is being assessed by an Aged Care Assessment Service.


What is a Community Aged Care Package?

A Community Aged Care Package is a package of care services that is delivered to you in your home. To access this service you must first be assessed by an Aged Care Assessment Service (ACAS). For information about your local ACAS contact Carelink Centres on 1800 052 222.

Services could include:

  • assistance with your meals, dressing, washing and laundry;
  • assistance with maintaining your health;
  • some maintenance/gardening services;
  • temporary respite care; or
  • social activities.

It is important to know that the services are very flexible and can change as your needs change.


What is an Extended Aged Care at Home Package?

Extended Aged Care at Home Packages (EACH) are very flexible and depend on the complexity of the care needs of the individual. To receive an EACH package you must be assessed by an Aged Care Assessment Service as needing high level care. Generally a person who requires high level care could be eligible for an EACH package, and the types of services that may be provided as part of an EACH include:

  • registered nursing care;
  • care by an allied health professional such as a physiotherapist, podiatrist, or other type of allied health care;
  • personal care;
  • transport to appointments;
  • social support;
  • home help; and
  • assistance with oxygen and/or enteral feeding.

We can:

  • help you to enforce your rights;
  • explain the services provided;
  • negotiate with the service provider about something you are unhappy about;
  • provide information and education to staff and management;
  • consult on policies to enhance consumer rights;
  • promote community awareness of the rights of older people.
